AE37, a therapeutic vaccine, is being developed by the Company's wholly owned subsidiary, Antigen Express, Inc. ( www.antigenexpress.com), for a number of indications in oncology. The Phase I trial concluded the vaccine was safe and well tolerated in breast cancer patients.
The vaccine is derived from a fragment of the HER-2/neu oncogene, which is over-expressed in a variety of tumors, including breast, ovarian, prostate, colon and lung tumors. The Phase I breast cancer trial was conducted by Dr. George Peoples at the Walter Reed Army Medical Center.
The completed Phase I trial was a dose escalation study to establish the safety and immunologic activity of AE37 in 15 node-negative breast cancer patients. The patients received 6 immunizations at monthly intervals of 100, 500 or 1000 mcg. AE37 combined with 0 to 250 mcg. GM-CSF as adjuvant. Because of a robust local response in patients receiving 500 mcg. AE37, patients were dosed with 1000 mcg. AE37 in the absence of adjuvant. Even in the absence of adjuvant, two of the three patients receiving 1000 mcg. AE37 required a reduction in the amount of AE37 because of a robust local response, indicating the inherent potency of AE37. Analysis of T cells taken from patients as well as delayed-type hypersensitivity tests demonstrated that there was a significant and specific immune response in patients receiving AE37. A full report, entitled "Results of the First Phase I Clinical Trial of the Novel Ii-Key Hybrid Preventive HER-2/neu Peptide (AE37) Vaccine," has been accepted for publication and will appear in the July issue of the Journal of Clinical Oncology.

I found evidence that new data for the AE37 vaccine will be presented at the 2010 American Society of Clinical Oncology Annual Meeting to be held in Chicago from June 4-8, 2010. The title of an AE37 abstract being presented at ASCO 2010 is Effect of a Novel Ii-key Hybrid HER2/neu Peptide AE37 Vaccine with GM-CSF as Compared to GM-CSF Alone on Levels of Regulatory T-cell Treg Populations. The listed author/speaker is Dr. Guy T Clifton, who has worked extensively on peptide based therapeutic vaccines at the USMCI with Antigen Express collaborator Col. George Peoples.
If Dr. Clifton highlights Phase II data that confirms previously reported Phase I data regarding AE37's effect on regulatory T-cell Tregs, that would be great news for Antigen Express. The Phase I trial for AE37 in breast cancer patients showed that Tregs were reduced in all patients tested pre- to post-vaccination. This smaller Phase I trial also found that DTH responses increased in all patients from pre- to post-vaccination p<0.0001. Boding well in this trial for the vaccine was that there was an inverse relationship between the degree of Treg reduction and the size of DTH response to AE37. The researchers concluded in this trial that the reduced levels of Tregs in vaccinated patients appear to be associated with more robust responses in ex vivo immune assays and in vivo DTH reactions suggesting that the AE37 vaccine may be clinically useful.
This pending update follows the impressive interim results for the AE37 vaccine in its current Phase II trial which were unveiled at the San Antonio Breast Cancer Symposium in December 2009. Two hundred disease-free, high risk, breast cancer patients who had completed standard adjuvant therapy are being enrolled and randomized to receive six monthly inoculations of either the AE37 peptide vaccine or the adjuvant GM-CSF alone to compare their ability to prevent cancer recurrence. This interim analysis followed the first 49 patients in the AE37 group and the first 71 for the control group. At a median follow up of 13 months, there were no (0.0%) recurrences in the AE37 group of patients, 0/49, compared to 7.0%, 5/71, in the control group.

New US patent bolster Generex's intellectual property assets

Generex Biotechnology Corporation, leader in drug delivery for metabolic diseases through the inner lining of the mouth, has received a new patent in the United States.
The Director of the United States Patent and Trademark Office has granted the company a patent titled, "Method for Administering Insulin to the Buccal Region", which can be viewed at www.uspto.gov.
This new patent increases the number of patents related to the company's proprietary buccal drug delivery platform technologies to 158 issued patents, 22 of which are United States patents. Generex has a total of 105 patent applications pending in various jurisdictions around the world.
"We continue to bolster the company's intellectual property protection with each new patent issuance," stated Rose C Perri, the company's chief operating officer. "The issuance of patents in key markets like the United States strengthens the company's intellectual property assets and is timely as our flagship product, Generex Oral-lyn, is available under the US FDA's expanded access Treatment IND programme
